Irei contextualizar minha dúvida. Na construção da documentação de um pacote, quando colocamos, por exemplo, # @param x descricao de x, o Rstudio destaca cada um desses termos com cores diferentes. Tem como fazer isso com o # TODO: ? Eu coloco e ele fica da mesma cor que comentários, seria melhor se ele tivesse um destaque. Se ele possue, como ativá-lo?

Não imaginava que isso fosse possível, mas, depois de pesquisar um pouco, descobri que estava enganado! O RStudio tem suporte para colorir palavras-chave (TODO e FIXME) em comentários, basta olhar no próprio código-fonte da IDE.
O problema é que nem todos os temas usam essa funcionalidade 
O Tomorrow Night Bright, por exemplo, é um dos poucos que faz isso (apesar de a cor não ser a mesma do @param):
Felizmente, desde a versão 1.2 do RStudio, é fácil criar um tema personalizado que atenda à sua necessidade. Ao que parece, você está usando o tema Dracula (assim como eu 
), então o que precisamos fazer é encontrar o arquivo dracula.rstheme no nosso computador, criar uma cópia em outro lugar e adicionar um comando simples baseado no código do Tomorrow Night Bright:

Para concluir a criação do nosso tema customizado, precisamos trocar o seu nome na primeira linha e editar o nome do arquivo em si (Buffy e buffy.rstheme no meu caso). Não sei se os arquivos dos temas são iguais em todos os sistemas operacionais, mas a minha versão ficou assim:

Bizarramente, eu não encontrei os arquivos rsthemes no meu computador (MacOS), não tem em nenhum lugar!!! Talvez seja por conta do computador ser da empresa ou algo assim, depois verei isso.
O importante é que consegui adicionar essa informação, mantive o mesmo nome de tema em homenagem, apenas mudando a cor, ficou mais interessante um destaque diferente entre @param e TODO.
Fiquei pensando, vale a tentar subir um Pull Request simples acrescentando essa mudança para o tema Dracula? Ou será que essas coisas já estão bem definidas o suficiente?

Eu já estive errado sobre esse tipo de coisa antes, mas acho que uma PR como essa não seria aceita. O tema Dracula tem uma definição formal para cada aplicação suportada, então você provavelmente precisaria passar pelo crivo do autor do tema primeiro. Na minha opinião, essa seria uma adição legal! Então talvez você devesse tentar mesmo se a chance for baixa.
P.S.: Eu não fazia ideia disso, de verdade, mas o autor do Dracula é brazuca! Ele chama Zeno Rocha e trabalha na WorkOS. Olha só o que acontece quando você tenta comprar o Dracula PRO no site dele:
